This book was first published in 1978. A second edition came out in 1980. By 1990, it had sold 200,000 copies worldwide.

In 1996, the trade-paperback version was released. This edition was distributed by Midpoint Books. Two years later, in 1998, the major publishing house Putnam picked up the book and published it through Perigee Books, a division of The Berkley Publishing Group, a member of Penguin Putnam, Inc.
